Transition Profile and Scope & Sequence
The ISD 917 Transition Profile is from the MN Disability Hub and is used throughout county agencies. It is based on the MN Transition Framework, also found on the MN Disability Hub. The profile works in conjunction with the Transition Scope and Sequence which has the curriculum and resources available to you at ISD 917 for you to use to teach the transition skills identified as needs on the profile.
The transition profile is based on the MN Transition Framework and aligns to the 917 Scope and Sequence. You can use it as an informal tool to gauge where a student's transition skills are and then look to the Scope and Sequence document to find resources that will help you address the student's specific needs.
FY27 ISD 917 Transition Scope and Sequence
The 917 Transition Scope and Sequence organizes the most popular 917 curriculum and resources based on the topic domains from the Minnesota Transition Framework. These topic domains include My Best Life, Independent Living, and Post-Secondary Education and Training. The scope and sequence is arranged by the learning stages outlined in the Minnesota Transition Framework including Awareness, Exploration, Preparation, and Implementation. You are welcome to submit your own lessons to add to the Scope and Sequence to share with your colleagues.
