Evaluation Procedures
Starting the Evaluation
Per federal guidelines, the district has 30 school days to complete an evaluation once consent is received.
Day 1 of the testing period is either:
- The day after a PWN is signed at an evaluation planning meeting
- The day after a signed PWN is returned to the school by the parent/guardian
- The 15th day after the form was mailed
- 15th day must be a school day
- If it’s not, testing period begins on the next school day after the 14th day.

Sending home a PWN
- IEP Manager tracks 14 calendar days from mailing date
- Contact parent around Day 7 and Day 10 if no response
- Site Admin Asst. will notify IEP Manager if PWN is received via mail
- Make sure Admin Asst gets signed form if it comes back in student’s backpack
- Add date and finalize in SpEdForms:
- When form is signed and received OR
- Once 14 days have elapsed
Beginning the Testing Period
On Day 1, the IEP manager is responsible for emailing the entire evaluation team to notify them that the testing period has begun.
Include:
- Student Full Name
- Date PWN consent was received (or date of implied consent)
- Deadline for each team member to submit their results to School Psychologist (Day 23)
- Final Deadline (Day 30 or the meeting date, whichever comes 1st)

Give Notice of Team Meeting for evaluation results meeting to Lead Teacher

All data and write-ups are complete in SpEdForms

Submit any remaining protocols or other paperwork to the Administrative Assistant

School Psychologist will complete their own sections, consult with the team regarding the Eligibility Determination section, and organize the Needs and Adaptations sections

Day 27-Day 30:

This should be your target date for the evaluation results meeting

1-2 days before meeting:

IEP Manager reads and checks the entire report for final grammar/spelling edits and any missing pieces (checking against PWN)

Print copies of the draft report for the meeting

You think a section is missing and needs to be added; or

You have information to add but you don’t know where it belongs
