All name changes require a copy of legal documentation to be sent with this form.

Legal documentation need only verify the name change. A copy of the entire document is NOT needed. Changing your marital status above does NOT change your taxes. You must do that on the EAC. You can also print out the W-4, and MN W-4, complete and send it with your form.

All other changes to your personal information, including tax withholdings, can be completed by you on the Employee Access Center (EAC). Directions to access the EAC can be found on the District website or in the Staff Handbook. You may also email HR@isd917.org should you have difficulties accessing the EAC

Current ISD 917 staff can earn UP TO $500 for referring new employees to our district!

What You Need to Know:

• To be eligible for a referral bonus, you must submit your referral information via the ISD 917 Online Referral Form before the prospective employee's first day of employment.
• The referral program applies to new hires only.  Rehires are not eligible.
• For start dates between July - November:
  • $250 if the new hire works for 90 calendar days
  • $250 if the new hire works for an additional 90 calendar days
• For start dates between December - June:
  • $250 if the new hire works 90 calendar days (or until the end of the school year)
  • $250 if the new hire continues employment at ISD 917 the next school year
